Campaign Disclosure Statements Please refer to the Fair Political Practices Commission Manual 2 included in your packet. Candidate without a Campaign Committee: If a candidate does not plan on having a campaign committee and does not anticipate receiving or spending $2,000 or more during the calendar year, he/she must file with the City Clerk’s Office a Campaign Statement Short Form (Form 470). ■ If a candidate expects that he/she will not spend or receive funds in excess of $2,000 and files a Form 470 and later does spend or receives more than $2,000 - the candidate must, within 48 hours, send written notification, (the Form 470 Supplement) and notify each candidate seeking the same office. Regular mail may not be used. Send by FAX, guaranteed overnight delivery or personal delivery. The candidate must also file a Form 410 Statement of Organization within 10 days of receiving over $2,000 in contributions. After a candidate has filed a Form 470 and then subsequently receives contributions (including monetary or non-monetary contributions, loans and the candidate’s personal funds) totaling $2,000 or more or makes expenditures totaling $2,000 or more, is required to send written notification (the Form 470 Supplement ) within 48 hours to the City Clerk, the office of the Secretary of State and to each candidate seeking the same office. Regular mail may not be used. Send by FAX, guaranteed overnight delivery or personal delivery. The candidate must also file a Form 410 Statement of Organization within 10 days of receiving over $2,000 in contributions . Candidate with a Campaign Committee: A candidate who intends to receive or spend more than $2,000 must open a campaign committee bank account and file a Statement of Organization (Form 410) with the Secretary of State’s Political Reform Division, and the City Clerk’s office. The Form 410 must be filed within 10 days of receiving $2,000 in contributions. The original and one copy of the form are filed with the Secretary of State and two copies are filed with the City Clerk’s office. The Secretary of State will issue the committee an identification number (ID); this ID number must then be used on each campaign statement filed thereafter. ■ Before a candidate for elective office can solicit or receive contributions, he or she must file a Form 501 (Statement of Intention). The law requires candidates to file an original and one copy of this form with the City Clerk’s Office. ■ File Form 410 within 10 days of receiving $2,000 contributions.
